Online Forms

The Online Forms screen allows you to view, save, and print forms your pension fund has made available to you. Additionally, some electronic forms (forms you fill out right on your computer screen) may be available to you from this screen.

To access the Online Forms screen, click on the Forms option in the menu. The Online Forms screen opens.

Online forms are either downloadable forms or electronic forms. All forms available to you are displayed as cards on this screen. The name of the form is displayed at the top of the card. The type of form is also indicated on the card.

Downloadable Forms: Clicking on the card displays a message instructing you to print and complete the form, then return it to your retirement office for processing. Click OK and indicate whether you want to open or save the form. Note that you need the ability to open Adobe Acrobat (.pdf) documents. Once the form is open, you can then save it locally or print it.

Electronic Forms: Clicking on an electronic form card displays a message instructing you to fill out the form by typing the requested information in the fields provided. Click OK and the form opens for you to fill out. Once completed, select Submit to send the form to your retirement office for processing. For electronic forms, it is not necessary to print the form or mail it to the retirement office. It is sent to the retirement office electronically.
